Volunteer Commitment

FILC is volunteer-run. Every family is asked to contribute one volunteer role per season, or buy out of the commitment.

Assistant Coach — work with the head coach at practices + games. Background screening required. Team Parent — coordinate team comms, snacks, end-of-season gifts. General Volunteer — game-day help, field setup, fundraising, or wherever the club needs a hand. Volunteer Buy-out (+$25) — decline volunteer commitment this season.

PHOTO USE RELEASE FORM

I hereby grant and authorize Fleming Island Lacrosse the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing.

I understand and agree that these materials shall become the property of Fleming Island Lacrosse and will not be returned.

I hereby hold harmless, and release Fleming Island Lacrosse from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.

Participation Waiver *

Participant Waiver Form

The PARTICIPANT agrees to assume the risk, responsibility and liability incidental to participating in this activity FLEMING ISLAND LACROSSE on the Property/Facility listed below, and to fully and completely defend, indemnify and hold harmless Clay County, Florida, and all of its officers, employees, agents and principals (collectively referred to as Releasees) against any and all liabilities, claims, actions, damages, costs or expenses asserted against the Releasees arising out of the provision by the USER attending/participating in above event on the Property/Facility listed above, and to pay all costs incurred in defending the Releasees against the same, including but not limited to attorney's fees and disbursements at both trial court and appellate court levels.

Medical Authorization *

In the event of a medical emergency where I cannot be reached, I authorize Fleming Island Lacrosse Club staff and coaches to obtain medical treatment for my child, including first aid, transportation to a medical facility, and emergency medical procedures as deemed necessary by qualified medical personnel.

Payment Plan / Scholarship
